OpenAI starts reporting unexpected model behaviour in public

On 16 September, OpenAI published a framework for reporting unexpected or concerning model behaviour and released six examples it had observed during the previous six months. It says it intends to disclose events sooner, including before every cause or mitigation is fully understood.
Most teams only learn about an AI workflow failure when a person spots a bad answer or an awkward handoff. That makes the lesson easy to lose, especially when the same prompt, context or tool use returns in another workflow. A visible incident record turns unexpected behaviour into a review input rather than a private anecdote. It gives the owner a way to decide whether to tighten instructions, remove an action, add an approval step or pause the workflow while the evidence is checked.
Analysis
For one recurring AI workflow, add a short incident record: the task, the unexpected behaviour, the evidence, the immediate containment and the rule you changed. Review it with the workflow owner before expanding access or autonomy.
