Without one decision record, the same folder questions are repeated in meetings and risky assumptions can reach the migration task. The tracker makes the decision, owner, evidence, and next step visible without adding code or automation.

Three fictional rows prove the normal, blocked, and excluded routes. The normal row reaches verified with dates and evidence. Each saved view returns the expected rows. A non-owner access check passes, and the Leadership update view exports the intended rows and columns to Excel.
